Program Overview

TLC Wellness is a physician-directed lifestyle and metabolic program offered through TLH, designed to help adults achieve sustainable weight management, improve energy, and restore long-term metabolic balance. The program is led by a board-certified physician in Obesity Medicine, Gastroenterology, and Internal Medicine, with additional training in Endocrinology at the Mayo Clinic and Clinical Nutrition at the University of Chicago, ensuring a medically grounded, whole-body approach to metabolic care.

 

Each participant begins with a comprehensive baseline evaluation and laboratory workup to rule out secondary causes of obesity and assess for metabolic comorbidities such as thyroid dysfunction, insulin resistance, or fatty liver disease. Patients may participate in person or via telemedicine, depending on convenience and location. The program combines individualized medical guidance, home self-monitoring, and expert-led exercise support through a private online community, all backed by evidence-based care. Participants receive personalized assessment, behavioral counseling, nutrition therapy, and medication management when appropriate, including GLP-1 receptor agonists and other proven pharmacotherapies.

 

The goal is simple: to make healthy, lasting change achievable and supported, every step of the way.

The program is led by a board-certified obesity medicine specialist and supported by a multidisciplinary team including a certified personal trainer, nutrition professionals, and behavioral health resources as needed. Each participant receives a structured, individualized care plan with clear milestones, ongoing follow-up, and direct physician communication for guidance, motivation, and accountability.

Enrollment & Cost

Initial Evaluation – $150
Comprehensive consultation and baseline laboratory screening to identify metabolic factors and rule out secondary causes of obesity. This visit establishes a personalized treatment plan, wellness goals, and safe medication options.
Lab testing included in the initial evaluation.

Monthly Subscription – $80/month
Ongoing physician oversight, nutrition and lifestyle planning, medication management when appropriate, and structured follow-up visits available in person or by telemedicine.

Optional Exercise & Accountability Community – $30/month
Access to the TLC Wellness Facebook Group, featuring guided weekly workouts, group challenges, peer accountability, and expert trainer feedback, keeping participants motivated and supported between visits.

Transparent, physician-supervised, and evidence-based — TLC Wellness makes expert metabolic care accessible, flexible, and personalized.

Prescription medications are not included in the program cost.

Transplant Coordination & Continuity of Care

Pre-Transplant Evaluation

Our team provides comprehensive pre-transplant assessments in close coordination with each patient’s designated liver transplant program. This includes laboratory testing, advanced imaging, cardiac and infectious disease clearance, nutrition optimization, and psychosocial evaluation. We maintain active collaboration with regional transplant centers, allowing flexible, shared-care arrangements tailored to patient preference.

 

Once the transplant program initiates its surgical evaluation, required in-person visits are completed at that center, while many psychosocial and medical updates may occur via telemedicine. Patients can continue longitudinal management with our clinicians for stabilization and coordination, or transition fully to their transplant center’s care, whichever best supports continuity and convenience.

Post-Transplant Follow-Up

The primary transplant program manages the early post-transplant period, including immediate recovery and immunosuppressive stabilization. When appropriate, long-term follow-up may transition to our care team, providing ongoing medical management, laboratory review, and coordination with the transplant center.

Our clinicians focus on comprehensive post-transplant wellness, including medication safety, infection prevention, cardiovascular risk reduction, and weight management, while maintaining direct communication with each patient’s transplant team. Telemedicine visits are available for stable patients to reduce travel and ensure seamless continuity of care.

Liver Imaging & Procedures

FibroScan® & Liver Stiffness Assessment

We offer FibroScan® for non-invasive measurement of liver stiffness and fat content (fibrosis and steatosis). Each study follows standardized protocols for accuracy and is performed by trained staff. Results are explained in clear, patient-friendly language with individualized follow-up recommendations. Screening focuses on people at risk for fatty liver, alcohol-related disease, viral hepatitis, or metabolic conditions. When results indicate concern, patients are guided promptly to clinical evaluation and appropriate next steps.

Ultrasound & Diagnostic Imaging

On-site ultrasound services include detailed liver and biliary evaluation with Doppler assessment of blood flow. We coordinate directly with radiology partners for advanced studies when needed, ensuring rapid reporting, accurate interpretation, and timely review with your clinician.

Phlebotomy & Laboratory Services

Our on-site phlebotomy suite provides convenient, same-day blood collection for both routine and specialized testing. All specimens are processed using temperature-controlled handling and documented chain-of-custody procedures to maintain safety and accuracy. Samples are transported to accredited reference laboratories for high-quality analysis that supports both clinical and research programs.
